Complete Doctoral Survey Form - Ph.D. Students Only

The Survey of Earned Doctorates (SED) is an annual census of all research doctorate recipients for U.S. educational institutions. Data gathered is used by various federal sponsors to make decisions about the levels and uses of resources available for future doctorate recipients.

Federal law requires that individual reports be kept confidential and prohibits the release of information that would identify any individual's responses.

The survey is conducted by the National Opinion Research Center (NORC) at the University of Chicago on behalf of the National Science Foundation (NSF), the National Institutes of Health (NIH), the U.S. Department of Education (ED), the U.S. Department of Agriculture (USDA), the National Endowment for the Humanities (NEH), and the National Aeronautics and Space Administration (NASA).

Beginning Spring 2016, completion of the survey is mandatory prior to initial submission of your dissertation.
