Assistantship Forms

Assistantship positions will not be awarded until you have been admitted into your graduate degree program. Individual academic and non-academic departments and units are responsible for award decisions, detailing the expected duties and responsibilities, stipend amounts, and establishing a work schedule. The assistantship application must be submitted to the office where you wish to be employed.

Degree Program Forms

[ Master's ]

Should be created first semester of enrollment and kept in department; due to OGS with Graduation Checksheet. All committee members must have a graduate faculty appointment.

Should be attached to Committee Request form and kept in department until paperwork is sent to OGS with Graduation Checksheet

Should be created as soon as research is approved and kept in department until paperwork is sent to OGS with Graduation Checksheet

Should be turned in to department by student at beginning of semester prior to graduation. After departmental review, form is due to OGS no later than end of that semester.

Should be created first semester of enrollment and kept in department; due to OGS with Graduation Checksheet.

Must be attached to Graduate Program of Study form if needed to list courses in excess of 10 that are on the original Graduate Program of Study form.

Should be attached to Program of Study form and kept in department until paperwork is sent to OGS with Graduation Checksheet.

Should be submitted to OGS as soon as transcript is available so courses can be reviewed and transferred.

This completed form must be sent electronically to Ms. Pam Sullivan, Manager, Graduate Programs, at least two weeks prior to the scheduled date.

[ Educational Specialist ]

Should be created first semester of enrollment and kept in department; due to OGS with Graduation Checksheet. All committee members must have a graduate faculty appointment.
